Revised MMS Form 131
This form is a revised version of MMS Form 131
to collect 1998 Performance Measures Data.
Here are a few highlights from the new form:
This form was sent out with NTL 99-N03 the first
week of March 1999.
1. MMS Operator Code - Participants
are asked to list all companies and corresponding operator codes for which their data
represent. If needed, MMS' list of
active companies and operator codes can be downloaded.
2. Calculations - Participants will
no longer be required to perform simple calculations (i.e., adding contractor and company
hours worked) especially since MMS is providing data for some measures.
3. Fax & Email Info - To improve
communication efficiency we ask for your fax number and email address.
4. Noncompliance vs. Exceedence - We
are now asking for all noncompliances of the National Pollution Discharge Elimination
System (NPDES) that are reported on Discharge Monitoring Reports (DMRs). We originally
asked for exceedences only. Exceedences are a subcategory of noncompliances and both
appear on DMRs.
