An accident occurred recently on an offshore mobile drilling rig, resulting in fatal
injuries to one man.
The victim was standing on a platform behind the crane, observing casing offloading
operations. As the crane swung inboard, the victim was caught by the rear of the crane and
swept between the crane and the rig floor housing (clearance less than six inches). To
avoid a recurrence of this type of accident, the operator has taken the following steps:
1. A railing has been installed around the platform to keep anyone standing on the
platform clear of the crane.
2. An audible alarm has been installed to warn that the crane is turning.
