of Mexico Region Injuries – 2002 (total 29 incidents, 47 injuries) 11-Jan-2002 - Union Oil Company of California
Remarks: Contract rigger fell 10 feet from temporary quarters. Rigger had been placed into the upstairs quarters by a crane basket and was in the process of pulling electrical wires. He opened the door on the second level and apparently lost his balance and fell through the doorway to the level below. The employee suffered a broken finger, lacerations to the head, and fractured vertebrae. He was taken to a local hospital. 16-Jan-2002 – Unocal Exploration Corporation
Remarks: A contractor floorhand dropped a valve. The valve fell and landed on the foot of the floorhand. The floorhand was treated and flown in for further evaluation. 29-Jan-2002 - Chevron U.S.A. Inc. (also listed as Fire)
Remarks: A worker went to the Master Control Panel to investigate why the system (natural gas) pressure was low. He opened up the back of the panel to troubleshoot the loss of pressure, in an attempt to locate the gas leak and correct the problem. The worker checked the panel vent ports and found no leakage. At about this time, he felt heat (hot flash) to his face and arms. He immediately shut the panel doors to contain the heat. Apparently the fire was brief and self extinguished immediately due to lack of fuel. The worker was then evacuated by helicopter for medical care. Remarks: The rig derrick hand was guiding a spacer spool to the wellhead when his left hand was caught between the spool piece and the wellhead flange. He has several broken bones in his left hand. 19-Feb-2002 - El Paso Production Company
Remarks: A lift boat capsized. Nineteen people were taken to the hospital for hypothermia and injuries. Two people were treated for injuries and the other 17 released. A minimal amount of pollution originated from the capsized lift boat during the first day, but no pollution was observed thereafter. 22-Mar-2002 - Shell Offshore Inc.
Remarks: A contractor roustabout fell 22 feet from a motorized personnel (scissor lift) basket on to the riser storage deck below. He suffered multiple fractures to a leg. Apparently the gantry crane operator inadvertently swung the crane into the basket and caused the injured person to fall out. 12-Apr-2002 - Newfield Exploration Company
Remarks: A worker slipped when getting up on the handrail to rehook the chain falls to install a generator. 16-May-2002 - BP Exploration Inc.
Remarks: The drill crew was attempting to move the main bushing. One of the floormen was injured when his right foot slid under the bushing just as the operator lowered it. 21-May-2002 - J. M. Huber Corporation (also listed as Fire)
Remarks: A fire occurred when natural gas was released from the seal joint of a flange, and was ignited by a cutting torch. The fire watch extinguished the fire. The operator received minor burns to his lips and cheeks. The operator was treated with burn cream and returned to work. 22-May-2002 - Shell Offshore Inc.
Remarks: While conducting a load test on a boat davit, the safety mechanism malfunctioned allowing hook to be pulled into the davit breaking the cable. There were two men in the boat at the time and both were injured. Later they were both put on a helicopter and sent to a hospital. 27-May-2002 - Anadarko Petroleum Corporation
Remarks: While attempting to straighten out a bottom hole assembly (BHA) on a coil tubing unit, the BHA shifted, severing a contract crewman’s right index finger. The injured crewman was taken to a local hospital and the end of his finger was reattached. 01-Jun-2002 - ChevronTexaco Corporation
Remarks: While rolling HWDP stacked on top of 16 inch casing an employee attempted to hold a loose chock in place with his hands. As the second joint was rolled toward the man, the chock shifted allowing the two joints of HWDP to continue toward the employee. As the man attempted to back away, the joints continued to follow him into the inboard way, one joint striking and pinching his left foot between HWDP and 16: casing, resulting in a broken leg. 01-Jun-2002 - ChevronTexaco Corporation
Remarks: While moving Drill Collars (DCs) from the casing bay port side to the port side DC rack using the port FWD crane, a bundle of DC’s (2) were set into place. The slings were removed. The employee noticed that the DC’s were not lying flat on the stripping and decided to try to straighten them out with the use of a pinch bar (approximately 5’ in length). The employee standing on the deck installed the pinch bar four to five feet from the box end and below the (8 inch) DC that was up approximately 4 inches above the stripping. While he was trying to pry the DC by using upward force, the DC slipped down to the tripping, causing the pry bar to strike him on the left arm just above the wrist. Remarks: While picking up the electric line pulley to position it at the crown level, another person then grabbed the air tugger (hoist) cable with his right hand. His hand was then pulled into the air tugger pulley, causing injury to his fingers. 14-Jun-2002 - Anadarko Petroleum Corporation
Remarks: The injured party was in the process of putting a new bag in the trash compactor and while closing the door on the front of the compactor, he fractured his little finger on his left hand. 15-Jun-2002 - Chevron U.S.A. Inc. (also listed as Explosion)
Remarks: An explosion occurred and five personnel were injured. Three of the contractor employees were flown into Terrebonne General Hospital in Houma. The contractor personnel were attempting to clean out a separator at the time of the incident. 16-Jun-2002 - Chevron U.S.A. Inc. (also listed as Collision)
Remarks: A M/V struck the platform causing multiple injuries to personnel on board. Twelve personnel were on board at the time of the collision. The injuries range from minor cuts and abrasions to serious back and neck injuries. The collision only caused minor handrail damage to the platform. 22-Jun-2002 - Chevron U.S.A. Inc.
Remarks: A minor injury occurred when elevator links shifted onto a floorhand’s foot. The employee was sent to shore by helicopter. 13-Jul-2002 - J. M. Huber Corporation
Remarks: A rigger was hooking up on a vent pipe to be lifted by a crane. After the crane began lifting, the rigger was attempting to throw the tag line overboard when the tag line wrapped around his arm. The rigger was lifted off the platform by his arm. After the crane operator put the back on the platform, it was determined that his arm was either fractured or strained. 01-Sep-2002 - Shell Offshore Inc.
Remarks: The sub-sea engineer was opening the doors on the BOP RAM with a hydraulic wrench when one of the control lines developed a pinhole leak. This high pressure liquid penetrated his glove and was injected into his hand, in the palm area, just below the small finger.
