Remarks: An employee was preparing to board a helicopter during a crew change. As he neared the helicopter, he stood straight up and was struck by one of the main rotor blades. He was evacuated to the hospital, where he died. May 18, 2001 Shell Offshore Inc.
Remarks: Initial reports indicate that during crane operations, a load shifted and resulted in the death of a rigger. The incident is under investigation by the U.S. Coast Guard. May 31, 2001 Shell Deepwater Production Inc.
Remarks: Initial reports indicate that a roustabout was crushed between a watertight door and the door facing. The U.S. Coast Guard is responsible for investigating this incident. July 31, 2001 The William G. Helis Company
Remarks: Initial reports indicate that the well began flowing through the drill pipe and casing valve. All personnel were evacuated, except for one missing crew member.
