March 20, 2001 – Anadarko Petroleum Corporation

Investigation: Complete Activity: Development
Lease: G02045 Event(s): Fatality
Area: East Cameron Operation: Production
Block: 270 Cause: Human Error
Rig/Platform: Platform B Water Depth: 175 feet

Remarks: An employee was preparing to board a helicopter during a crew change. As he neared the helicopter, he stood straight up and was struck by one of the main rotor blades. He was evacuated to the hospital, where he died.


May 18, 2001 – Shell Offshore Inc.

Rig/Platform: Brutus TLP site Water Depth: 1890 feet

Remarks: Initial reports indicate that during crane operations, a load shifted and resulted in the death of a rigger. The incident is under investigation by the U.S. Coast Guard.

Remarks: Initial reports indicate that a roustabout was crushed between a watertight door and the door facing. The U.S. Coast Guard is responsible for investigating this incident.

Remarks: Initial reports indicate that the well began flowing through the drill pipe and casing valve. All personnel were evacuated, except for one missing crew member. The person was still missing after two weeks and presumed dead. This incident is under investigation by MMS.

Remarks: Preliminary reports indicate that a pipe rack finger fell from the derrick, killing one employee and injuring another. This incident is under investigation by MMS.
