Mexico Region Fatalities 2000 May 19, 2000 BP Amoco Corporation
Remarks: On the morning of May 19, 2000, a contract employee was using a portable winch (come-along) to remove a section of removable guardrail for the purpose of accommodating the installation of the mounting beams of a temporary crane. One end of the come-along was attached to the section of guardrail to be removed, while the other end was attached to a section of fixed guardrail located approximately 5 feet above the guardrail to be removed. As tension was applied to the come-along, the upper guardrail failed at its base welds and detached from the deck. As a result of the detachment, the employee fell approximately 60 feet to the plus 10 deck and sustained fatal injuries. August 16,

Remarks: On the
evening of August 16, 2000, a drilling contractor employee was air-hoisted into the
derrick to assist another employee secure a stand of drill pipe that had fallen across the
derrick from the fingers, and had rested on the monkey board. At a height of approximately
80 above the rig floor, the employees body harness/boatswains chair became
detached from the hook of the air hoist line. The employee fell to the rig floor and
sustained fatal injuries. See OCS Report MMS
2000-089 September 8, 2000 - Exxon Mobil Corporation
Remarks: During routine drilling operations on the MODU Marine 700, the riser cart/carriage assembly moved forward, pinning a drilling floorhand between the riser cart/carriage assembly and the iron roughneck on the drill floor. The floorhand received fatal injuries as a result of this accident. October 27, 2000 Ocean Energy, Inc.
Remarks: The employee was found unconscious on the motor room floor. It appears that a piece of equipment fell and struck the employee. The U.S. Coast Guard is responsible for investigating this incident. November 11, 2000 Energy Partners, Ltd.
