Remarks: A M/V was tied to the rig when the M/V apparently drifted into the rig. The bow of the boat collided with the starboard area of the rig’s main deck. The deck plating had an area three foot by one foot that was damaged and ten electrical cables were severed. The rig returned to normal operations after repairing the damages and investigating to ensure there as no structural damage to the rig.

Remarks: A M/V collided with the platform on the north side. The vessel went under the platform and struck the no. 9 well conductor bell guide. There was also some slight damage to some handrails. The vessel was a commercial fishing vessel and had no injuries.

Remarks: A helicopter was destroyed when it impacted an offshore platform and water following a loss of control during takeoff from the EI 188 P platform, located in the Gulf of Mexico.

Remarks: While continuing offloading operations, the M/V experienced a brief and sudden loss of power form the bow thruster. The loss of power caused the vessel to drift toward the platform and allowed contact between the vessel and platform. 

The captain terminated the water transfer operations immediately and attempted to use the vessel’s main propulsion to counteract the vessel’s drift toward the platform. Without bow thruster power, the main engines could not overcome the seas, current and wind, inching the vessel toward the rig. Once the vessel lay alongside the platform, the platform disconnected the stern lines and hoses, and the vessel was able to pull away under her own power.   The platform sustained minor damage.

Remarks: A M/V struck the platform causing multiple injuries to personnel on board.    Twelve personnel were on board at the time of the collision. After the collision, personnel on the M/V were rescued from the sinking vessel, water, and platform. The injuries range from minor cuts and abrasions to serious back and neck injuries. The collision only caused minor handrail damage to the platform.

Remarks: While attempting to tie up to the platform A structure, the wind and seas pushed the M/V into the platform. The platform and the pipeline were shut-in and inspected. The pipeline suffered only minor paint damage and was slowly brought back on-line, after the boat was freed from the platform. The platform suffered mostly handrail and walkway damage. No injuries reported.

Remarks: Personnel at the platform observed a sheen emanating from platform. The platform was immediately shut in using Supervisory Controlled Data Access (SCADA) system, stopping any release of hydrocarbons. 

Upon arriving at platform, operator personnel discovered that an unknown boat had apparently struck the platform. One wellhead was bent to approximately at 25-degree angle, causing a flange below the master valve of the wellhead to leak hydrocarbons. Two other wellheads had needle valves, nipples, and gauges broken off the wellheads, causing additional hydrocarbons to be released. Estimated damage amount is $107,000.

Remarks: A shrimp boat struck Caisson No. 12, and a lift boat. Caisson no. 12 is bridge connected to the larger CC platform. The vessel caused minor damage to the Caisson and the lift boat. The lift boat jacked down and moved to a safe location. No injuries or pollution reported.
