Remarks: A 200 bbl temporary pressure tank was being used to catch CaZnBr2 completion fluid from the test separator system with the inlet of the tank temporarily connected to the test separator water dump valve. The BPV and PSV on the pressure tank were piped into the 16" high pressure vent line with a 4" ball valve downstream of the PSV. With the 4" ball valve closed during this process, the pressure tank overpressured causing the inspection cover to blow off subsequently striking the flash gas pressure fuel filter. The velocity of the pressure released caused the pressure tank to move approximately 15' striking a beam connected to the 12-3/4" gas meter run causing damage to both the beam and meter run. During this incident approximately 3 to 4 bbl of crude oil and 80 bbl of CaZnBr2 fluid was lost from pressure tank which was located over open grating.

Remarks: Sixty-three mile long, 1-inch steel tubing that transports methanol from WD143A to subsea manifold/wells developed a leak. Seas 5' to 7', winds 20 knots. The operator has stopped pumping and mobilized a ROV to survey the line.

Remarks: The relays for the SSV and 2nd SSV were pinned out of service. Therefore, when the field was shut in and the pressure cascaded to the satellite structure, the PSH's tripped, but the wells did not shut in.
