Remarks: A 2" weld neck flange and valve to a 4" reducing tee on a subsea tie-in was snagged by a fishing net or boat anchor. Response: cap was welded over the 2" opening and sand bags were placed over the top of the 4" pipeline tie-in. The pipeline connects between ST300 Platform A to SS241 Platform A. Remarks: A 1" valve protruding from a threadolet on an 8-5/8" spool piece on Exxon's subsea tie-in on oil pipeline to Texaco's 20" EIPS was apparently pulled from the threadolet by a trawl net. Response: deployed Clean Gulf Associates Fast Response Unit; shut down EIPS; divers and surveying equipment were dispatched; removed section containing the broken valve and blind-flanged the ends of the pipe remaining on the sea floor. Remarks: Workover rig. While bringing a well on line after service, a dump line developed a leak. Zinc bromide (15.7 ppg) leaked through the dump valve for the fluid holding tank into an overboard line. Response: the dump valve has been repaired, and the overboard line has been welded shut so that fluid has to be pumped out. Lost fluid valued at $27,000.
