Remarks: A submerged 6" gas & oil pipeline from SS04A to SS145B developed a pinhole leak while line was being purged to abandon. A pollution spill occurred before the pipeline could be shut in. Response: the platform was shut in, and the Fast Recovery Unit was activated.


November 13, 1986 (2 days) - Mobil Oil Exploration & Producing Southeast I

Cause: Leak & Human Error: coupling & judgement
Distance to Shore: 96 Miles Water Depth: 750 Feet

Remarks: Hydraulic fluid was released from leaking hydraulic couplings during underwater pile driving operations. Six separate incidents happened during a two day period.

Pipeline Segment: 03137 Pipeline Diameter: 8 Inches
Other Events:

Remarks: A submerged pipeline between platforms SP65-A and SP62A leaked from 13" long crack. The cause is thought to be either anchor dragging or initial construction damage casing material fatigue. Response: a 36'joint of pipe was cut out and replaced using hydrocouples.
