Remarks: The tank bottom on a belted holding tank ruptured allowing discharge into the Gulf.

Remarks: The diesel tank was overfilled during refueling operations. The rig personnel requested 4,000 gallons but the supply boat pumped 8,200 gallons. The supply boat crew, rig mechanic, and crane operator were all absent during refueling operations.
