Remarks:  There was a gas release due to a rupture of a Ύ -inch diameter piece of pipe attached to the glycol vessel located on Platform Hidalgo’s Production Deck. The glycol vessel was operating at normal pressure of approximately 1285 psi with normal H2S concentrations of approximately 19,500 ppm. The release was initially detected by a combustible gas detector which triggered an alarm and a simultaneous emergency shut down (ESD).

Remarks:  There was a gas release due to a pinhole leak in an idle 3-inch line formerly used to route the condensate.  This line had not been used for approximately 6 years since an alternate route for the condensate was installed.

Remarks:  While raising the 13-5/8-inch drilling riser and positioning it to be placed in the “V” door the two nylon slings broke.  The riser fell and struck the northwest corner of the rig and pipe deck.

Remarks:  There was a gas release occurred from the lower man way.  Improper series flexitaulic gaskets were installed on both tope and bottom of man ways.  Only a small portion of the flexitaulic material was actually making a seal.
