Pacific Region Other Incidents - 2003 (5 total) May 4, 2003 – Aera Energy LLC
Remarks: The crane operator picked up the equipment basket and swung the load around to be in position to load the basket onto an approaching M/V. The operator then swung the load back to load additional items. In doing so, he felt the clutch slip and he started to lose control of the load. He pushed the block and winch line levers down to stop the boom’s descent; however, the boom did not stop descending until the load hit the water. The boom impacted platform railings, grating, and associated support structures and sustained substantial damage. The load remained attached to the crane boom. Remarks: Drill line failure while RIH with tubing for sand clear out operations resulted in traveling blocks falling and impacting the rig floor. Two joints of tubing, rig floor grating, and grating support structures were damaged. Well is certified as "no flow" and was secured by pipe rams and stabbing valve. Remarks: A 480 V power cable was pinched between a railing on the mud dock and a crane load being lifted, causing a small arc which tripped a breaker and knocked out platform power. The power cable was replaced and rerouted to avoid a reoccurrence. November 2, 2003 – Exxon Mobil Corporation
Remarks: During cable lay operations, the cable pulling head assembly separated from the power cable and approx. 625' of cable was dropped on the seafloor. The cable was retrieved using an ROV and pulled back onto the cable lay vessel undamaged. The fallen cable did not impact other operating pipelines or power cables associated with the SYU operations.
