Remarks: While laying down drill pipe, a 32-foot joint of 3.5 inch drill pipe weighing 4200 lbs slid out and landed in the Gulf. Plans are to retrieve it when the structure is removed and site clearance is done. There were no injuries and no pollution from the incident. 10-Feb-2005 BP Exploration & Production Inc.
Remarks: The crane operator had repositioned the tool box to make room for placement of a wireline unit. While attempting to swing the eastside crane around to retrieve the unit, the crane fast line became entangled in the west side (seatrax) cranes' walkway. When the fastline became free, the hook and ball swung in the direction of the helicopter. The crane operator attempted to "pull up" on the fastline but was unable to prevent the hook from colliding with the helicopter main rotor and tail fin. 18-Feb-2005 GOM Shelf LLC
Remarks: While conducting a load test on the crane, the crane boom failed. The crane boom, water bag and dynometer fell to the water. The boom was severely damaged but there were no injuries. 18-Feb-2005 Apache Corporation
Remarks: A Bell 206-L-3 model helicopter went down 200 yards off the platform with three persons on board. The seas were 2 3 feet at the time of the incident. All personnel were recovered safely after spending forty minutes in the 72-degree water. No one was seriously injured. FAA Investigated the incident. 19-Feb-2005 Tana Exploration Company LLC
Remarks: Operators at WC 71, A quarters platform noticed a large gas cloud at WC 102 Platform 28. Personnel dispatched to the scene discovered that a pipeline flange gasket had failed. 21-Feb-2005 Freeport-McMoran Energy LLC
Remarks: Two load blocks and 1 crane cable were dropped overboard. The crane cable snagged on a platform beam cutting the cable. Each block was 1-inch diameter and weighed 75 lbs. 03-Mar-2005 Apache Corporation
Remarks: A barge carrying East Cameron block 265 platform D topsides was headed for protected waters due to weather when the topsides were lost overboard. Remarks: A Rotocraft Bell L-3 landed on ST 220 and the pilot reported to Rotocraft that he felt a shutter in the aircraft. The helicopter subsequently fell off the heliport into the water. Debris was scattered on the heliport and platform. One of the rotor blades struck a tote tank located on the lower deck. The pilot emerged from the helicopter 10 -15 seconds after it hit the water and was transported to the hospital by the US Coast Guard. 13-Mar-2005 Marlin Energy Offshore, LLC.
Remarks: While the LB Lauren was jacking down from platform, a crack was discovered in the can of the leg tower which left only one of the five planetary gears teeth fully engaged. Remarks: On March 15, 2005 during a BOP function test, the slip joint hydraulic packer failed spilling 19 bbls of SBM. 17-Mar-2005 Apache Corporation
Remarks: Fast line on rental crane, used for construction broke while lifting two 20 feet x 8-inches spools of pipe bolted together. The spools hit the cargo building tool house and fell into the water. 22-Mar-2005 BP Exploration & Production Inc.
Remarks: Ensco 82 was moving on the platform when it sheared off a 5-foot piece of handrail. The handrail was not retrievable. 05-April-2005 Millenium Offshore Group, Inc.
Remarks: While lifting waste heat unit from the platform to the boat, the sling parted. The unit fell a small distance on the boat. 30-April-2005 Forest Oil Corporation
Remarks: The An Air Log Bell 407 landed on SS 277 A to refuel and pick up one passenger for transport. After refueling the helicopter, the electrician removed the nozzle, replaced the cap, moved the hose back to the port side and locked the nozzle. He then kicked the hose back to its original position, near the helideck skirting and parallel to the helicopter skid. Upon takeoff, with light rain and winds gusting around 30 mph, they felt a jerk, nosed down and then the pilot recovered control. When the helicopter reached the next platform, personnel noticed the helicopter with the hose hanging from its side. 02-May-2005 W & T Offshore, Inc.
Remarks: During the jack down operations, the port and stern legs of the jack-up boat were set down on Transcos 24-inch gas pipeline, SN12579. The pipeline was not buoyed and reflectors were not installed. Remarks: A section of handrail measuring 3.5 feet x 3.5 feet with 3 rails of 2 pipe was hit while lowering a diverter spool in place and knocked overboard. 16-May-2005 Chevron Texaco Corporation

Remarks: During off loading operations using the crane, a rack containing ten sections of chicsan pipes was pushed off the heliport when a 25 feet X 5 inches hose basket was lifted off the heliport. The hose basket slid 5 feet when being picked up. The operator inadvertently pushed the chicksan pipe off, crushing the TV antenna and bent a handrail before falling in the Gulf. 07-Jun-2005 Walter Oil & Gas Corporation
Remarks: While placing a drum in a drum rack, the tip of the crane boom made contact with a portion of a wind spoiler attached to the port aft leg, causing it to break loose and fall into the Gulf waters. The piece was approximately 12 feet long and weighed approximately 25 pounds. The spoiler is constructed of fiberglass and sank immediately after hitting the water. 04-Jul-2005 Ridgelake Energy, Inc.
Remarks: On July 4, 2005, Ridgelake Energy was forced to stop work on substructure installation due to increasing seas in advance of Tropical Storm Cindy. Remarks: A 23,000 lbs winch was being removed from under deck during hurricane preparedness. As it was being moved, the lifting beam and monorail parted, causing the winch to fall overboard. No damage was reported to the hull or pontoons. 22-Jul-2005 B P Exploration & Production Inc.
Remarks: On well A-3, the telescopic joint weighing 23,000 lbs that connects to the BOP stack fell 25 feet from below the drill floor onto the BOP stack. The telescopic joint is held up by a slip joint with four screws. The slip joint was rated for 100,000 pounds. After the telescopic joint settled, it was found that all four screws had sheared off. The slip joint, telescopic joint, and the BOP adapter spool were damaged in the incident. There were no other damages, no loss in production and no injuries. 25-Jul-2005 Bois dArc Offshore Ltd.
Remarks: While picking up a pressure washer, the hydraulic cylinder that raises and lowers the crane boom came apart. The crane boom fell over backwards. There were no injuries and no pollution associated with this incident. 31-Jul-2005 Spinnaker Exploration Company, LLC.
Remarks: A 27 ft. section of 9-5/8-inch mouse hole broke off and fell from the rig, striking the bladder housing of the master pneumatic surface safety valve which spun the bladder housing 180 degrees. The pipe continued on through the grating of the production tree deck striking the casing gauge and the SCSSV actuator. This bled off the hydraulic pressure and shut down the platform. The section came to rest on the well head deck, wedged between the outside of the deck and handrail, and was then securely fastened and retrieved from under the rig floor. Remarks: All power was lost on the drilling rig top drive, mud pumps, and drill floor while drilling Well No. 3 at 17700 feet. Changing out the PLCs restored power to the top drive, mud pumps and drill floor. Rig power was lost for 45 minutes. All rig components were checked for proper operations. Drilling resumed from 17700 to 17911 feet. An increase in gas units was detected and the well shut in at the sub sea BOPs. Gas breaking out of solution in the riser was circulated out resulting in approximately 10 bbls. of non-hydrocarbon, synthetic mud being discharged overboard. The riser was conditioned and the integrity of the mud system and hole stability verified. Remarks: Cable from the drill floor tugger became tangled in the traveling block. When the block was lowered, the tugger cable broke and fell about 226 feet on top of the tugger. There were no injuries. 05-Oct-2005 Apache Corporation
Remarks: During crane operation a helicopter landed on the heliport. The crane operator got out of the crane after securing load. The helicopter was powering down, and the crane operator then got back into the crane and tried to move the load. When he did so, the fast line (rope) on the crane came in contact with the main rotor. There tips of four rotor blades appeared to be slightly damaged.
