Remarks: While in the process of changing out portable 1000 KW generators, a rigger was pinned between the generator and a tank. He sustained a broken rib and several cracked ribs. 03-Mar-2005 Venoco, Inc.
Remarks: A contract worker was transferring between the platform and M/V using a rope swing. The vessel was apparently rising and falling with the passing swells. When he swung over the railing, the vessel was riding in a trough which momentarily increased his height above the deck. Unable to slide down the knotted rope, he released his grasp on the rope and fell to the deck sustaining a compound fracture to his right ankle. 19-Mar-2005 Plains Exploration & Production Company (PXP)
Remarks: A pool well services employee sustained an open wound to the palm side of the right ring finger while tripping pip out of the hole. He was one of two employees guiding the Hawk Jaw Unit onto a stand of drill pipe in preparation for breaking the joint. Another employee was operating the winch that reels the unit in and out. When it was determined that the jaws were not going to line up properly, the winch was stopped causing the unit to abruptly torque counter clockwise. Remarks: While walking down the stairs from the production deck to the 14 foot level, the contractor slipped on the wet stairs and landed on his back sustaining an injury to his back. He was carrying a large cooler filled with water samples at the time and not holding onto the handrails. He was transferred from the platform to the boat using a Billy Pughe. He subsequently was treated by a physician and placed on 2 weeks restricted duty. 24-Aug-2005 Dos Cuadras Offshore Resources, LLC
Remarks: A contractor was working in the tower (monkey board) when he stepped on a bolt fastened to the floor and rolled his ankle. He experienced swelling and pain and was transported to the hospital where he was diagnosed with a mild ankle sprain and placed on 4 days restricted duty. 18-Sep-2005 Dos Cuadras Offshore Resources, LLC
