Remarks:  A contract electrician fell approximately 20’ while running cable in an overhead cable tray above the drill floor.  He was transported to the hospital via medivac helicopter for treatment of two broken legs, two broken ankles, and fractures to his left foot.

Remarks:  A contract employee fell from the rafters approximately 10-12’ while installing new pipe in the pipe rack. The worker had previously disconnected his fall protection gear to crawl between two beams.  As he was attempting to tie off his landyard to the beam, the pipe shifted and he fell from the pipe rack.  He was transported to the hospital via medivac helicopter for stitches and further observation.

Remarks:  One gallon of produced water containing a small amount of crude oil was released into the water due to a broken nipple on the produced water line.  The spill resulted in a barely visible sheen (5 yards x 167 yards).  An employee was sent to the hospital with minor injury to his leg from hot water.

Remarks:  A contract employee was climbing up scaffolding to enter the spider basket.  He was wearing a full body harness, life jacket, and other PPE but was not properly tied off.  He stepped onto a plank that had not been properly secured causing the plank to rock back and forth.  He apparently lost his balance and fell approximately 35’ to the water below.  He was able to swim to an access ladder and lift himself onto the platform.  He sustained minor scratches and bruising, and was placed on light duty the next day.
