Remarks: An electrician was pulling cable into an electrical cabinet when the cable became grounded creating an arc and flash fire. The electrician sustained burns to his left hand, upper chest and lower neck area. The electrician was medi-vaced to a hospital.

Remarks: While pulling tubing, a gas bubble escaped through and around the tubing string discharging oil and water onto the rig floor and drill deck area. The first worker was injured when his safety glasses were knocked off and the discharged fluids got into his eyes. He slipped and fell while trying to activate the remote shutdown panel, banging his knee. Approximately 1 to 2 hours later, while in the process of cleaning the deck area, the second injury occurred when a second worker slipped and fell while exiting the stairs from the drill deck to the rig floor injuring his hip and knee.

Remarks: A painting contractor was climbing a ladder holding a gallon of paint. The ladder was not tied off or held by another worker. The ladder was propped against the mud tank. The ladder slipped causing the paint to fall 10 inches striking the equalizer line on the mud tank. He complained of pain in his tailbone and numbness in his left leg. He was medi-vaced to the local medical center were he underwent treatment.

Remarks: A diver was working in the splash zone cleaning marine growth. The diver was holding the trigger of the blaster gun in one hand and the barrel in the other hand. When the diver released the trigger lock, the initial blast of discharge blew into the palm of his right hand causing a 3” gash in the palm below the thumb. The diver was transported to a local hospital.

Remarks: The rig motorman backwrapped a spinning chain on a stand of HWDP to make it up. A driller pulled in breakout ton line attached to the spinning chain to make up connection. As the stand was making up, the motorman put his gloved right hand on the cable and caught the fingertip of his glove in the wirerope loop where the cable connects to the spinning chain. The cable ripped off his glove and bent his fingers back. The injured party was transported by crewboat to shore. The injured party dislocated his ring and middle fingers and received lacerations to his right hand.
